Use only black ballpoint pen to fill out forms. Certificates must be created within three months.
Submit a copy of your admission ticket to the Examination for Japanese University Admission for International Students for which you are requesting results.
(The following two forms are submitted only by applicants for the School of Health and Sport Sciences)
Entrants admitted as international students will receive 30 percent discount on the standard tuition fees for 2010. The 30 percent tuition discount for 2011 and thereafter may be changed upon consideration of the economic condition at that point. Details of tuition and fees are for 2010, and may be changed due to economic conditions.

It is important to take care of your health while studying in Japan. If you feel sick in class, you can go to the Health Center for treatment. Just in case, be sure to enroll in the National Health Insurance program. In addition, for the subscribers of the National Health Insurance Program, a subsidy up to 80 % (It will be changed to 35% from April 2006) of the treatment fee paid can be received upon request if you are registered in the "Foreign Students Medical Subsidy System" by the Association of International Education, Japan.
Some students return to their own countries, others continue to graduate school, while others aim to gain employment, etc, but the International Center and Careers Center will offer consultation and assistance to all.
There are sure to be many differences in lifestyle and culture between your country and Japan. If you have any problems in academic or everyday life, feel free to consult the International Center, Student Affairs office, or Careers Center on campus.
In Chukyo University, staff members who have taken classes given by the Ministry of Justice and who are licensed by the Immigration Office for application by a representative handle various application forms required by the Immigration Office for you. (It is called application by an agent, or "TORITSUGI SHINSEI.") Applications for renewal of residential permits and for activities outside of visa qualifications are, generally, taken care of in full at the university.
